Early life nutrition, the developing immune system and subsequent allergic manifestations

چکیده
Over the last half-century, chronic disease incidence, including allergic manifestations (AM), has increased exponentially in developed nations and amongst the wealthy in developing nations. Concurrently, there has been a “nutrition transition” in the same populations. Dietary patterns have shifted from a prudent diet (high fruit and vegetable-, and omega-3 fatty acid intake; low omega-6 fatty acid intake) to a western-style diet (higher levels omega-6 fats, lower amounts of fresh fruits and vegetables, and omega-3 fats). While diet can impact health at any life stage, it is likely to be most important during the fetal and infancy periods, when the immune system is immature. Based on an extensive review of the literature, we considered this hypothesis for the 3 nutritional progressions from fetal life through infancy: prenatal nutrition, breastfeeding and introduction of solid foods. While there is little conclusive evidence to support that a prudent diet is protective against AM especially after birth, modest evidence suggests that a prenatal maternal nutrition high in n-3 and micronutrients (e.g. vitamins C, D and E) protect against AM. This indicates that there may be a window of opportunity during fetal development in which allergic manifestation risk may be altered. The effect of breastfeeding on the prevention AM is unresolved. Despite this, exclusive breastfeeding to 4-6 months should be encouraged as human milk contains numerous immunological properties and is associated with other health benefits. If breastfeeding is not possible, hydrolyzed formulas seem to confer a greater protective effect than traditional formulas. Solid foods should be introduced at age 4-6 months. The order of introduction seems to have little effect on AM. At present, it is impossible to conclusively state that a particular dietary pattern or food will alter AM risk in infants. Implications for research and practice are also discussed.

A Review of Primary Immunodeficiency Diseases with Skin Manifestations
Introduction: Primary immunodeficiencies (PID) are rare heterogeneous disorders with defects in which one or more components of the immune system are malfunctioning. Clinical presentations of the patients according to type of immunodeficiency are variable. The majority of these patients are susceptible to infections depending on the type of disorder. In these patients, one of the most important...
متن کاملAtopic dermatitis: global epidemiology and risk factors.
Atopic dermatitis (AD) is a chronic inflammatory skin disease posing a significant burden on health-care resources and patients' quality of life. It is a complex disease with a wide spectrum of clinical presentations and combinations of symptoms. AD affects up to 20% of children and up to 3% of adults; recent data show that its prevalence is still increasing, especially in low-income countries....
متن کاملIs there a role for fatty acids in early life programming of the immune system?
There may be a causal relationship between n-6 PUFA intake and allergic disease and there are biologically plausible mechanisms, involving eicosanoid mediators of the n-6 PUFA arachidonic acid, that could explain this. There is some evidence that high linoleic acid intake is linked with increased risk of atopic sensitisation and allergic manifestations. Fish and fish oils are sources of long-ch...
متن کاملEarly exposure to a nonhygienic environment alters pulmonary immunity and allergic responses.
The hygiene hypothesis suggests that early life exposure to a nonhygienic environment that contains endotoxin reduces the risk of developing allergic diseases. The mechanisms underlying the hygiene hypothesis are unclear and may involve subtle immune system interactions that occur during maturation. Experimental objectives of this study were to use a novel animal model to test the hygiene hypot...
متن کاملEarly-life environmental determinants of allergic diseases and the wider pandemic of inflammatory noncommunicable diseases.
The unparalleled burden of a diverse range of chronic noncommunicable diseases (NCDs) is a major global challenge in the 21st century. Chronic low-grade inflammation is a common feature of virtually all NCDs, indicating a central role of the immune system. Furthermore, as the most common and earliest-onset NCD, the epidemic of allergic diseases points to specific vulnerability of the developing...
